We welcome you to our practice and thank you for choosing our office for your medical care. As a valued patient it is important to us that you become familiar with some of our office policies.

  1.  We ask that you check in with the receptionist upon arrival at each appointment.
  2. If your insurance requires you to pay a copay it is due at time of service. We do not bill for copays. If you are unable to make your copay at that time you may be asked to reschedule, with the exception of emergencies.
  3. It is your responsibility to verify insurance coverage for certain procedures.
  4. Cancellations made at least 24 hours in advance allow us to accommodate you in the case of a re-schedule and fill an available appointment time with other patients desiring care. If a cancellation occurs without 24 hour notice, please be aware that you are responsible for the appointment day charges.
  5. All accounts are to be paid in full each month. We may be willing to make payment arrangements for you depending on your account balance and other circumstances that may apply.

For your first appointment, we will send you a new patient information packet. This packet needs to be filled out and mailed back to Stone Medical, P.C. before coming into the office.

PLEASE BE AWARE: If you are given a patient packet (of any kind), we ask that all pages are returned TWO DAYS prior to your scheduled appointment. If you are unable to return them for review, we will accommodate a re-schedule. We thank you in advance for your cooperation.
